Photo ID and insurance cardsA valid photo ID and all current insurance cards.

Payment informationAny deductible or co-insurance due. If you don't have insurance, payment is due at the time of service.

List of your medicinesInclude doses, allergies and any recent surgeries.

Previous imagesIf you had the same exam somewhere else, bring the CD and report so we can compare.

Your formsFill out your patient forms at home to save time, or arrive 15 minutes early.
